The whole thing in plain words
We collect what we need to sign you up, work out your fee, and get your bills paid — your contact details, your date of birth, your insurance details, and your payment details. We share it with the clinics and your insurer so your membership works, and with the companies that host our systems and take your payment. We do not sell it. You can ask us what we hold, ask us to correct it, or ask us to delete it. Your actual medical records belong to the doctors who treated you, not to us.
About your medical records. Medical records and protected health information created by the doctors, clinics, and facilities that treat you are maintained by those providers and are governed by their own privacy practices and by laws such as HIPAA. This Policy covers the information we collect through our Services — not your providers’ medical records.
This Privacy Policy explains how Instinctive HealthPass LLC (“Company,” “we,” “us,” or “our”) collects, uses, and shares information about you when you visit instinctivehealthpass.com, request a quote or consultation, enroll in the Instinctive HealthPass membership program, or otherwise interact with us (collectively, the “Services”). By using the Services, you agree to this Policy.
1. Information We Collect
In plain words: what you type into our forms, what your browser tells us automatically, and what the clinics and your insurer tell us in order to get your bills sorted.
Information you provide. Name, contact details (email, phone, mailing address), date of birth, household/family member details for family memberships, insurance information (carrier, plan, and in-network maximum out-of-pocket), eligibility information, payment information, any Explanation of Benefits (EOB) or bills you submit to us, and any information you include in inquiries, consultations, or forms.
Information collected automatically. Device and usage data such as IP address, browser type, pages viewed, referring pages, and dates/times of access, collected through cookies, pixels, and similar technologies, including those provided by our advertising and analytics partners (such as Google and Meta).
Information from third parties. We may receive information from participating facilities and providers, your insurer, payment processors, and service providers in order to administer your membership and benefits.
2. How We Use Information
In plain words: to sign you up, price your membership, get your bills paid, take your payment, answer you when you get in touch, keep the systems running and safe, and do what the law requires.
- To enroll you, verify eligibility, calculate your membership fee, and administer your membership and benefits;
- To coordinate with participating facilities, providers, and your insurer regarding services and patient responsibility, including processing EOBs and bills you submit;
- To process payments and manage billing, renewals, suspensions, and cancellations;
- To respond to your inquiries and provide customer support;
- To operate, secure, and improve the Services, measure and serve advertising, and prevent fraud; and
- To comply with legal obligations and enforce our agreements.
3. How We Share Information
In plain words: with the clinics and your insurer, with the companies that run our payments and systems for us, and with the law when it asks. We do not sell your information for money.
We do not sell your personal information for money. We may share information:
- With participating facilities, providers, and your insurer to administer your membership and benefits;
- With service providers who perform functions on our behalf (such as payment processing, hosting, analytics, communications, and advertising), under contractual confidentiality obligations;
- With our affiliates for the purposes described in this Policy;
- When required by law, subpoena, or government request, or to protect the rights, safety, or property of the Company, members, or others; and
- In connection with a merger, acquisition, financing, or sale of assets, subject to this Policy.
4. Advertising, Cookies, and Analytics
In plain words: we use cookies to make the site work and to advertise. You can turn cookies off in your browser, though some things may stop working. If your state gives you the right to opt out of ad tracking, we honour it — including the Global Privacy Control signal.
We use cookies, pixels, and similar technologies to operate the Site, remember your preferences, measure performance, and support advertising. We advertise through platforms such as Google and Meta and use their tools on our Site, and we also advertise through offline channels such as television and radio. Depending on your jurisdiction, our use of advertising cookies and similar technologies may be considered a “sale” or “sharing” of personal information for cross-context behavioral advertising.
You can control cookies through your browser settings; disabling some cookies may affect Site functionality. Where required by law, we honor opt-out preference signals such as the Global Privacy Control (GPC). You may also opt out of advertising-related sharing by contacting us using the details below.
5. Your Choices and Rights
In plain words: depending on where you live, you can ask us for a copy of what we hold, ask us to fix it, or ask us to delete it. You can unsubscribe from marketing email with the link at the bottom of any of them, and stop texts by replying STOP. Asking will never get you treated worse.
Depending on your state of residence, you may have rights to access, correct, delete, or receive a copy of your personal information, and to opt out of any “sale” or “sharing” of your information and of targeted advertising. For example, residents of California and certain other states have these rights under their state privacy laws. You may also opt out of marketing emails (via the unsubscribe link) and texts (reply STOP). To exercise your rights, contact us using the details below. We will not discriminate against you for exercising your rights, and we will verify your request as required by law.
6. Data Security
In plain words: we protect your information with proper safeguards. Nobody honest can promise perfect security, so we do not.
We use reasonable administrative, technical, and physical safeguards designed to protect personal information. No method of transmission or storage is completely secure, and we cannot guarantee absolute security.
7. Data Retention
In plain words: we keep it while we need it to run your membership and to satisfy tax and legal rules, then we delete it or strip your name off it.
We retain personal information for as long as needed to provide the Services, comply with legal, tax, and accounting obligations, resolve disputes, and enforce our agreements, after which we delete or de-identify it.
8. Children’s Privacy
In plain words: this is for adults. If you have children on a family membership, you are the one responsible for their details.
The Services are intended for adults. We do not knowingly collect personal information directly from children under 13. For family memberships, the adult account holder is responsible for information about household members enrolled under the membership.
9. Communications and Text Messaging
In plain words: give us your number and we may call or text you about your membership. Reply STOP to stop, HELP for help. Your carrier may charge you for the message. If you work for us and switch on staff text alerts, that is your own choice, and your number is used only for those alerts. We never sell or hand anybody’s mobile number or texting consent to another company for their marketing.
If you provide your phone number, you consent to receive calls and text messages from us regarding your inquiry, account, or membership. Message and data rates may apply, and frequency varies. Reply STOP to opt out of texts and HELP for help. See also our Terms of Use.
Staff text alerts. Members of our staff may choose to receive automated text alerts about the leads they work — a reply from a lead, or a reminder before a booked call. Opting in is voluntary, is done by the staff member personally in the portal, and is not a condition of employment or of using the portal; it can be withdrawn at any time by replying STOP or by unticking the box. The mobile number on a staff record is used for those alerts only. The full terms are in our Text Alert Terms.
No sharing of mobile information. Mobile telephone numbers and text-messaging opt-in information are not sold, rented, or shared with third parties or affiliates for marketing or promotional purposes. The service providers that transmit messages on our behalf receive a number only to deliver the message.
10. Service Area and Third-Party Sites
In plain words: we operate in Arizona, mainly greater Phoenix. If we link you to somebody else’s website, their privacy rules apply there, not ours.
Our Services are directed to residents of Arizona, primarily the Phoenix metropolitan area (Maricopa County). Our Services may link to third-party sites and services that have their own privacy practices; we are not responsible for those practices, and we encourage you to review their policies.
11. Changes to This Policy
In plain words: if we change this, the new version goes up here with a new date, and every version we have published is listed at the bottom of this page.
We may update this Policy from time to time. When we do, we will post the updated Policy with a revised “Last Updated” date, and material changes will be communicated as required by law. The current version of this Policy is always available on our website and in your member portal, so you can review the most recent version at any time. By continuing to use the Services after changes take effect, you accept the updated Policy.

“Emailed to members” means the change touched what you pay or what your membership pays for, so every active member was sent an email about it. “Not needed” means it was a wording or contact-detail fix that changed nothing you agreed to.
